UNT Health Science Center, Department of Internal Medicine is looking for a full-time Assistant/Ass! ociate professor, non-tenure track, physician to serve as the Medical Director of the Discharge Planning for Medicaid Elders program. The Medical Director will be responsible for supervising teams of NPs/PAs and social workers and medical assistants who will provide medical care to Medicaid eligible elders (age 65+) and near elders (age 65+) with discharge planning. The medical director will work with local hospitals to ensure patients are referred to the program, in home assessments and follow-up occurs and return to their primary care providers or if none exists connect them with a new primary care practice for care.. The Medical Director will also provide direct medical care for patients in the program as well as UNTHSC Geriatrics clinics and programs. The Medical Director will be responsible for ensuring the program meets all metrics and will report directly to the PI. The Medical Director will also rotate into the UNT Health Science Center, Geriatrics call schedule (if! certified Geriatrician). There are opportunities to have TCOM! medical, as well as other UNTHSC health profession students' precept through clinics and discharge in home care program. Substantial academic opportunity exists through participation in program evaluation as well as teaching opportunities.
Required Qualifications
The successful candidate will possess the following required qualifications:
- DO/MD degree from an accredited college
- completed the three required years of training
- board certified and TX licensed by time of start date.
