Internal Medicine/Geriatrician Physician for the Dallas Home Based Primary Care (HBPC) Program.To

Recruitment/Relocation Incentives are authorized for this position.DUTIES: VA North Texas,
Dallas VA Medical Center is recruiting for a full time staff physician for an
active Home BasedPrimary Care (HBPC)Unit which treats approximately 400-450 in home patients
per year. The candidate will be responsible for the overall medical care
provid! ed to the HBPC team. The incumbent must work effectively with an
interdisciplinary team of healthcare professionals and be able to identify
medical problems and formulate appropriate management. He/She must make home
visits to assigned HBPC patients. In addition the candidate must collaborate
with HBPC physician assistants for care of patients. Candidates should have the
ability to provide effective care to patient; i.e. assesses, plans, implements,
documents, and evaluate patient care. Incumbent serves as Primary Care Provider for HBPC team.
